Dr. Binaya Basyal uses innovative defibrillator technology to provide a personalized treatment option for a patient with a life-threatening heart rhythm condition.
Virginia Cardiovascular Specialists (VCS) has achieved an important milestone in advanced heart rhythm care with its first implantation of the Medtronic Aurora Extravascular Implantable Cardioverter-Defibrillator (EV-ICD), performed by cardiac electrophysiologist Dr. Binaya Basyal on October 13, 2026.
The procedure provided a new treatment option for a 51-year-old patient with congestive heart failure and ventricular tachycardia, a potentially life-threatening condition that causes the heart’s lower chambers to beat abnormally fast.
The patient’s medical history made a traditional implantable defibrillator unsuitable, highlighting the importance of having advanced treatment options for patients with complex cardiovascular needs.
The patient had been experiencing episodes of ventricular tachycardia (VT), a dangerous heart rhythm that can prevent the heart from pumping blood effectively and may lead to sudden cardiac arrest.
An implantable cardioverter-defibrillator (ICD) can detect dangerous heart rhythms and deliver electrical therapy to restore a safer rhythm.
Traditional ICDs typically use thin wires, called leads, that travel through blood vessels into the heart. However, previous surgeries had limited this patient’s access to those blood vessels, making a conventional ICD unsuitable.
The patient also needed a device capable of delivering antitachycardia pacing (ATP), which uses small electrical impulses to interrupt certain dangerously fast heart rhythms, potentially avoiding the need for a defibrillator shock.
The Aurora EV-ICD offered an alternative that addressed both needs: protection against dangerous heart rhythms and specialized pacing therapy, without placing leads inside the heart or blood vessels.
“Every patient’s anatomy, medical history, and heart rhythm condition are unique. The EV-ICD gives us another important treatment option for patients who may not be candidates for a traditional defibrillator. Being able to deliver both defibrillation and specialized pacing to treat dangerous heart rhythms, without placing a lead inside the heart or blood vessels, is a meaningful advancement in personalized cardiac care.”

Unlike traditional defibrillators, the Medtronic Aurora EV-ICD uses a lead positioned beneath the breastbone, outside the heart and major blood vessels. The device itself is implanted beneath the skin along the left side of the chest.
This design allows the device to monitor and treat dangerous heart rhythms while avoiding certain complications associated with leads placed inside the heart and veins.
The system provides two important therapies:
While other defibrillators can be implanted without leads inside the heart, they do not all offer ATP. This capability made the Aurora EV-ICD particularly valuable for this patient’s treatment needs.
The Aurora EV-ICD represents an important advancement in treating patients at risk of sudden cardiac arrest, particularly those who may not be suitable candidates for conventional defibrillator systems.
For VCS, this first procedure reflects a continued commitment to bringing innovative cardiovascular technologies into clinical practice and providing treatment options tailored to each patient’s unique medical needs.
Congratulations to Dr. Basyal and the VCS care team on this important milestone in advanced heart rhythm care.
