Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) show real benefits for VCS Patients!

VCS began working with HealthSnap to implement a Remote Patient Monitoring program (RPM) for our Medicare patients in late 2021. Wanting to know the real impact of these programs on our patients, the VCS Board and physicians asked HealthSnap to conduct an analysis of any clinical benefit of the RPM program.  HealthSnap was able to review 629 patients who had participated in the program for at least one year and recorded blood pressure readings at least twice a week.  Our physicians have found the results impressive and believe it important to share them with our patients and the entire VCS Team:

  • 232 patients improved from uncontrolled hypertension to controlled. This is a 37% reduction in uncontrolled hypertension
  • At the beginning of the program 360 (57%) of the program, patients exhibited Stage 2 hypertension.  Reviewing the last 7 blood pressure readings for these patients only 200 patients remain in stage 2, a 25.2% reduction in patients with Stage 2 hypertension.
  • In the time since enrollment, 86.9% of patients improved systolic blood pressure, with an average reduction of -14.22 mmHg.
  • In the time since enrollment, 84.7% of patients improved diastolic blood pressure, with an average change of -7.57  mmHg.

The adoption of the RPM and CCM programs has proven to be a truly impactful and effective tool to improve the lives and health of our patients.  VCS looks forward to continuing these programs to help improve the lives of our patients!
